This Privacy Policy explains how Tribal Health, LLC (“Tribal Health,” “our,” “us,” or “we”) collects, uses, and shares your personal information, and the choices you have in connection with this. This Privacy Policy applies to the information collected on Tribal Health’s website (www.tribalhealth.com) (the “Website”), the information collected by Tribal Health through its third-party scheduling application (the “Scheduling App”), the information collected by Tribal Health through its third-party resume and credentialing optimization tools (“Rippin’ Resume” or “SNAP Resume”), and the information collected by Tribal Health through its in-person or other interactions with you (“Interactions”) (together, the “Services”).

Notice at Collection: At or before the time of collection, California residents have a right to receive notice of our practices, including the categories of personal information to be collected and the purposes for which such information is collected or used, your rights with respect to such information, whether such information is sold or shared, and how long such information is retained. INFORMATION WE COLLECT AND HOW WE USE IT

We collect personal information, which is information that identifies, relates to, describes, is capable of being associated with, or could reasonably be linked, directly or indirectly, with you, when you engage with our Services. Specifically, we collect personal information directly from you, from third-party sources, and automatically as you use our Website.

1. Information Provided By You

We collect personal information directly from you when we:

• Provide client services. When you choose to work with Tribal Health, we collect your identifiers(name, email address, physical address, and Social Security Number), customer records information (payment information), professional or employment information (employment and education history), and protected classifications (race, gender identity, veteran status, and disability status). We use this personal information to provide our Services, comply with legal and ethical obligations related to the provision of our Services, and to communicate with you about our Services.
• Register you for an event. When you sign up for a Tribal Health event, we collect your identifiers (name, telephone number, physical address, and email address) and your employment information(place of employment and job title). We use this personal information to keep a record of attendees, register you for the event, and contact you about updates regarding the event. We may also use your personal information to send you marketing communications about other events and topics we believe may be of interest to you.
• Host an event. When you attend a Tribal Health event, conference, or seminar, we collect your identifiers(name, email address, and telephone number) and your professional or employment information (employer, professional license number, job title). We collect this personal information to keep a roster of attendees and to send you marketing communications regarding other events that may be of interest to you.
• Respond to your questions or inquiries. When you contact us or send us any questions or inquiries, we collect your identifiers(name, email address, and telephone number) and any other information you choose to include in your correspondence. We use this personal information to respond to your questions or inquiries, troubleshoot where necessary, and address any issues you have with our Services.
• Publish alerts and newsletters. When you sign up to receive our newsletter or alerts, we will collect your identifiers (name and email address) and topics of interest. We use this information to send you the newsletter and/or alerts you request. We may also use the information to send you marketing communications on other topics we feel may be of interest to you.
• Assess your employment application. When you apply for employment with Tribal Health, we collect your identifiers(name, email address, physical address, and telephone number), and your professional or employment information (resume, cover letter, employment and educational history). We use this personal information to manage and consider your application with Tribal Health and as necessary to comply with the law. • Allow you to use the Scheduling App. When we allow you to use our third-party Scheduling App, we collect your email address, scheduling information (e.g., availability), clock-in/clock-out times, and logged hours. We use this information to manage your work schedule and attendance, calculate your hours worked, and for internal reporting purposes.
• Allow you to use Rippin’ Resume (aka SNAP Resume). When we allow you to use our third-party resume tool Rippin’ Resume (aka SNAP Resume), we collect the information you share with us in connection with the resume tool. We use this information to reformat and refine your resume into a format more compatible with that required by federal healthcare facilities.

Tribal Health may also use the personal information we collect as described in this section to comply with the law, to efficiently maintain our business, and for other limited circumstances as described in HOW WE SHARE YOUR PERSONAL INFORMATION.

2. Information Collected Automatically

In addition to the personal information you provide to us directly, we may also collect information from you automatically through the use of cookies and similar tracking technologies as you use our Website or the Scheduling App. This information may include the following internet or other network or application activity information:

• Cookie-based collection. This includes details of your usage, IP addresses, and other relevant data.

• Third-Party Links. Our Website may contain links to third-party websites, plug-ins, and applications. These connections may trigger the collection or sharing of data by third parties. While we do not control these third-party websites, we encourage you to review their privacy policies before interacting with them.

• Automated Data Collection. During your interaction with our Website, we automatically collect Technical Data about your device, browsing actions, and patterns using cookies and similar technologies.

• Third-Party Notices and Cookie Usage. Some cookies and tracking technologies may be deployed by third parties or by us for third parties, as described below. These parties may gather information about your activities on our Website, potentially linking it to your personal information, and may track your online activities across various websites and services with your consent.

• Usage information. This includes which pages of our Website you visit, the frequency of access, how much time you spend on each page, what you click on while using the Website, and referring website addresses. This also includes how you use the Scheduling App, such as the pages you visit and the actions you take within the Scheduling App.
• Device information. This includes certain information about your device that you use to access our Website and/or Scheduling App, such as browser type, browser language, hardware model, operating system, and your preferences.
• Location information. This includes information about your location, which may be determined through your IP address.

To collect this information, we use necessary, performance, analytics, and advertising cookies. We may use the collected information to: (i) conduct analytics to improve the Website and the Scheduling App; (ii) enhance user experience; (iii) provide you with targeted online advertising and measure the effectiveness of our advertising; (iv) prevent fraudulent use of the Website and the Scheduling App; and (v) diagnose and repair errors.

We aim to provide you with control over your personal information. While we do not currently respond to “Do Not Track” signals, to disable cookies and limit the collection and use of information through them, you can set your browser to refuse cookies or indicate when a cookie is being sent. Please note that disabling cookies may result in some parts of our Website becoming inaccessible or functioning improperly. Particular third-party cookies to note on our Website include:

• Google Analytics:To conduct analytics, we use Google Analytics. Google Analytics may set cookies on your browser or read cookies that are already there. Google Analytics may also receive information about you from applications you have downloaded that partner with Google. 3. Information Collected From Third Party Sources

Tribal Health may also collect personal information from other sources in the following situations:

• Publications and professional organizations. We may obtain your identifiersand professional or employment information from publications and professional organizations.
• Clients, employees, and business partner We may obtain your identifiersand professional or employment information from clients, employees, or business partners.
• Social media platforms. We may obtain your identifiersand professional or employment information from you when you interact with us on social medial platforms.
• Employment providers. We may also obtain your identifiers, professional or employment information,and commercial information from sources who conduct background screenings.

HOW WE SHARE YOUR PERSONAL INFORMATION

We share your personal information for the following purposes:

• Within Tribal Health. Where necessary, we share your personal information within Tribal Health as appropriate to provide our Services and to efficiently maintain our business.
• In the event of a corporate reorganization. In the event we enter into, or intend to enter into, a transaction that alters the structure of our business, such as a reorganization, merger, acquisition, sale, joint venture, assignment, consolidation, transfer, change of control, or other disposition of all or any portion of our business, assets, or stock, your personal information would be part of the assets shared with third parties, including the buyer or target (and their agents and advisors) for the purpose of facilitating and completing the transaction. We would also share your personal information with third parties if we undergo bankruptcy or liquidation, in the course of such proceedings.
• To prevent harm. We will share your personal information if we believe it is necessary to detect, investigate, prevent, or take action against illegal activities, fraud, or situations involving potential threats to the rights, property, or personal safety of any person.
• For legal purposes.We will share your personal information where we are legally required to do so, such as in response to court orders, law enforcement or legal process, including for national security purposes; to establish, protect, or exercise our legal rights, as required to enforce our terms of use or other contracts; to defend against legal claims or demands; or to comply with the requirements of any applicable law.

You can unsubscribe from our marketing emails by clicking the “unsubscribe” link provided in each email. Please note that we will continue to send you emails necessary to the Services you request.

SUPPLEMENTAL PROVISIONS FOR CALIFORNIA RESIDENTS

This section supplements our Privacy Policy and only applies to our processing of personal information that is subject to California Consumer Privacy Act, Cal. Civ. Code § 1798.100 et seq. as amended by the California Privacy Rights Act (the “CCPA”).

1. Collection of Personal Information

In the preceding 12 months, we have collected personal information from the following categories identified in INFORMATION WE COLLECT ABOUT YOU, which are described using the terms identified under the CCPA: identifiers, personal information, commercial information, internet or other similar network activity, professional or employment related information, and inferences.

2. Disclosure of Personal Information

In the preceding 12 months, we have disclosed personal information in the following categories: identifiers, personal information, commercial information, internet or other similar network activity, geolocation data, and inferences. Categories of third parties with whom personal information is disclosed include our IT support, our communication vendors, our mailing vendors, our client service providers, our CRM providers, our third-party integrated applications, and our employment screening providers.

3. Sharing and “Sales” of Personal Information

 In the preceding 12 months, we have not “shared” or “sold” (as those terms are defined in the CCPA) the personal information of California residents.

4. Collection of Personal Information from Children under 16

In the preceding 12 months, we have not knowingly collected or processed personal information pertaining to children under the age of 16.

5. Sensitive Data

 If we process sensitive data as defined by California law, we will only do so for the purposes specifically authorized by California law and in a manner that is necessary and proportionate for those purposes. As such, we do not perform any processing for which a Right to Restrict request is available.

6. Your California Privacy Rights

The CCPA grants certain rights to California residents. To the extent the CCPA applies to our processing of your personal information, you would be entitled to the following rights:

Right to Withdraw Your Consent	You have the right to withdraw your consent to our processing of your personal information. Please note that your withdrawal will only take effect for future processing, and will not affect the lawfulness of processing before the withdrawal.
Right to Opt-out	You have the right to opt-out of the processing of your personal information for the purposes of:(1)   Targeted advertising or sharing;(2)   The sale of personal information; and/or(3)   Profiling in furtherance of decisions that produce legal or similarly significant effects concerning you.At this time, we do not engage in targeted advertising or the sale of personal information, or decision-making based solely on automated processing, including profiling, which produces legal effects concerning you or which significantly affects you. Therefore, we do not offer rights related to profiling results.
Right to Non-Discrimination	You have the right not to receive discriminatory treatment for exercising the privacy rights conferred by law. We will not discriminate against you because you exercised any of your privacy rights, including, but not limited to, by: denying goods or services to you; charging different prices or rates for goods or services, including through the use of discounts or other benefits or imposing penalties; providing a different level of quality of goods or services to you; or suggesting that you will receive a different price or rate for goods or services or a different level or quality of goods or services.
Right to Appeal Your Decision	You have the right to appeal our decision if we decline to process your request. If applicable laws grant you an appeal right and you would like to appeal our decision with respect to your request, you may do so by informing us of this using the information provided in your request decision correspondence, or if that is not available, the information in the “CONTACT US” section below and providing us with information supporting your appeal.

For requests submitted via email, you must provide us with sufficient information that allows us to reasonably verify you are the individual about whom we collected the personal information and describe your request with sufficient detail to allow us to properly evaluate and respond to it. If we are not able to verify your identity with the information provided, we may ask you for additional pieces of information. You may choose a person or business that you authorize to act on your behalf to submit your requests (“Authorized Agent”). If you choose to use an Authorized Agent, we require that you provide the Authorized Agent with written permission to allow them to submit your request and that you verify your identity directly with us. Failure to do so may result in us denying your request.

RIGHTS OF NEVADA RESIDENTS

If you are a consumer in the State of Nevada, you may request to opt-out of the current or future sale of certain of your personal information. We do not currently sell any of your personal information under Nevada law, nor do we plan to do so in the future. However, you can submit a request to opt-out of future sales, as defined by Nevada law, by contacting us at privacy@tribalhealth.com. Please include “Opt-Out Request Under Nevada Law” in the subject line and body of your message.

HOW WE PROTECT YOUR INFORMATION

We implement and maintain reasonable security measures, such as encryption and access controls, to protect the personal information we collect and store. Our security includes physical, administrative, and technology-based measures. Please be aware that no method of transmission over the internet or method of storage is 100% secure, therefore, we cannot guarantee absolute security.

DATA RETENTION

We store personal information for as long as necessary to carry out the purposes for which we originally collected it and for other legitimate business purposes, including to meet our legal, regulatory, or other compliance obligations.

We determine how long to retain your personal information by taking into account various criteria, such as the type of Services provided to you, the nature and length of our relationship with you, possible re-enrollment with our Services, the impact on our Services we provide to you if we delete some information from or about you, and our obligations under applicable law.

You have the option to opt-in for extended data retention of your information collected via Rippin’ Resume or SNAP Resume and by opting in you agree to allow us to retain your resume information for up to one year (or longer if selected) beyond the initial application period. You can opt-in by checking the appropriate box during the resume submission process or by updating your preferences in your account settings.

DO NOT TRACK DISCLOSURE

We do not support Do Not Track (DNT).

CHILDREN’S PRIVACY

The Services are not intended for individuals under the age of eighteen. We do not collect personal information from any individual that we know to be under the age of eighteen except with the requisite permission of that individual’s parent or guardian.

UPDATES TO THIS PRIVACY POLICY

We may update this Privacy Policy. When we update this Privacy Policy, we will notify you by posting the revised Privacy Policy to this page with the effective date. If changes materially affect your privacy rights, we will notify you by prominent posting on our Website.
